We help you be a part of your community
Everyone deserves to form valuable friendships, pursue hobbies and explore their local community. Participating in the community is a great way to boost your self-worth, enhance your creativity and build independence.
Whatever your interests are, we will search for places that make your heart happy! From attending a craft group to Dungeons and Dragons, dance or music, day trips to explore or photography – the world is your oyster. We can offer one-on-one support or organise group activities to achieve the goals outlined in your NDIS plan.

Let’s strengthen your social and community participation together
Let us take the hassle out of community access. Share your interests, aspirations and goals with us, and we will find activities and events to help fulfil them. Check out our Events page for upcoming events. Our support team have either learned or lived experience, so you can trust that they know how to navigate all possible scenarios and provide compassionate assistance with social participation.
What we can do for you
We offer community access supports every day of the week, either regularly or as a one-off for a special event you have in mind. We can accompany you on your trips to the grocery store, the bank, medical appointments, community events or even family or friends’ houses – we are here for you.

Getting Started with Us is Simple

A 15-Minute Consult
This quick consultation will determine when and how we can help you. We will ask for basic details about you and your situation, along with any current NDIS details or plans you have so we can ensure we are the best people to support you.

Meet & Greet
Meeting you in person is super important to us as we want to ensure you feel safe and comfortable with your assigned support worker. As a smaller, close-knit team, you are always a top priority. No one gets left behind in the Your Kind Collective family.

Making an Action Plan
Once you have made a decision to work with us, we will map out a list of actions that we will go through to bring your plan to life. This helps us to keep on track and to prioritise tasks that need to be completed to connect you to the right supports.

Regular Check-Ins
Once you are fully connected, we will not disappear. We
will check in with you at regular intervals to make sure
you are still happy and that your needs are being met. As
your long-term support team, we are there with you throughout your entire NDIS journey. We perform regular
check-ins to ensure you’re feeling prepared, confident
and capable to face each day. We are always available to
answer any questions that may come up.
Frequently Asked Questions
About Support Coordination

What is Support Coordination?
Support Coordination is funded support provided on a case-by-case basis. This support means you will always have someone there to help you understand what your NDIS plan means (it can be a whole new language!), plan alongside you to find the supports that will best meet your needs, and connect you to those supports.
Once you have established your support network, a Support Coordinator will help you to maintain that network. Another vital role of a Support Coordinator is reporting back to the NDIA when the time comes for your NDIS plan to be reviewed. Above all else, a Support Coordinator is there for you and should always act in your best interests.
Is there a limit on how long I can be funded for Support Coordination?
Usually, Support Coordination funding will reduce over time. This is because a Support Coordinator should be assisting you in learning and building your capacity to navigate the NDIS independently. However, as long as you can demonstrate that you require assistance navigating your NDIS plan, Support Coordination should continue to be funded in your plan. If you feel you require Support Coordination funded in your plan, you can request it from the NDIA. Please contact us to learn more.
Is a Plan Manager the same thing as a Support Coordinator?
No. A Plan Manager is separate funded support and manages the payment of invoices from your other NDIS providers and tracks your plan expenses so that you can ensure your funding lasts for the duration of your NDIS plan.
